2 questions: Random resets of my file-associations and no toolbar

Question 1: When i click on a rar file, i have made a program (In VB.NET)
that gives me a selection wether i want to open it in BSPlayer (A media
player) or WinRAR (An archive handler)...

The problem is that sometimes, at any RANDOMLY given time, vista just
decides to reset that file association to its default, which is WinRAR...
For files that had no original default before i set them (Like .hdmov),
revert back to being unopenable... (I have to select which prog i want to
open it in)

Any idea how to fix that, its getting VERY annoying?
I mean, theres gotta be some way, because WinRAR's association seems to last
forever and ever... Something wrong with the windows association thingy?

Question 2: When a folder has a dot in its name, like this example:

Photos - 2008.02.21

Windows thinks its a .21 file, when its infact a FOLDER !!!!
The disappearing icon im talking about is the one on the blue toolbar on the
"Open" button...
I mean, how can the MS programmers miss something so fundamental?
Any fix for this as well?

Appreciate any help, especially for the first question... Reason why im
posting both is that i think the second has something to do with the first,
since Vista keeps cache'ing the .20, .21, .22 filetypes, making some kind of
overflow in association storage (Just a random guess)

I would suspect the BSPlayer has a bad application file.
