OK, here are the symptoms:

Using:
- Dell Axim X50 with Windows Mobile 2003 Second Edition v4.21.1008 (Build
14260.2.0.5)
- Active Sync 3.8 (Build 5004)
- Outlook 2003 (11.6359.6408) SP1 (All Updates Installed)
- Windows XP Home Edition Version 2002 SP2 (All Updates Installed)

I have hard reset the PPC numerous times...the initial ActiveSync seems to
work fine.
Future ActiveSyncs give a loooong "Looking for Changes..." follow by this
error message:
"Synchronization cannot be completed successfully. Disconnect your device
and try again. If the problem persists, see the Microsoft ActiveSync
Troubleshooter in Microsoft ActiveSync Help on your desktop computer."

On the PPC, everything seem to be fine except, none of my upcoming
appointments show on the Today screen and if I try to open Calendar, I get
the spinning windows circle in the middle of the screen...Calendar never
opens.

I've tried soft reset, it just brings up the Dell splash screen until I
finally hard reset. Never runs initial setup config stuff. (Tap stylus
here, set time zone, etc.)

I've run the PST repair tool on my outlook file.

I've Repaired my Outlook 2003 installation.

I've seen many similar problems in the Google Groups archive...but no
solutions that have worked.

Help!

Are you restoring a backup after the hard reset? If so, try not doing
so.
