Approved Updates don't get pushed to clients

Recently, I was made aware that the latest round (3/15 according to my last
Synch log) o9f updates had not been installed onto a workstation in my
network. Some were Office (Excel and Outlook) one was a Malicious Software
Removal Tool and two were Windows updates (KB912475 and Root Cert. Update).

I can understand why the Office updates didn't get downloaded, and can see
that Root Certificate Updates don't get downloaded, but I'm wondering why the
KB912475 didn't get installed even though it was synchronized and approved on
3/15.
