Can No Longer Update Via Run-As

Some time in 2005 I lost the ability to run Windows update via
right-clicking Windows Update and choose "run-as" administrator.

What happens is that it all looks fine but as soon as there is anything to
install (either updates or the update program itself) it will start the
downloads but fail to install. The only way to install it is to log off,
log on as the administrator, then run the update from there.

What would cause that and how do I fix?
(No I don't use automatic updates).
