Can't Change File Type Associations

First of all, a cross-post:

Summary of Problem:
Me, and one other guy (linky) can't get our file type associations to
change. I/we click "choose program," navigate to the EXE, double-click or
click "open," and it does not appear under the list of available programs.
Yes, I'm checking "always use this program." That is irrelevant, since the
selected program never shows up in the list after being selected in the first
place. Using Vista Home Premium x64, but I remember this happening back when
with XP 32, also.
