Open User Accounts in Control Panel and check your name. It should show you
as Administrator.
What version Outlook are you using? It's been a while since I've done any
forms, so bear with me.

Put in "forms permissions" and pick Outlook from the drop-down menu.
There are numerous Outlook newsgroups under Microsoft.public that may have
more help. Sorry I couldn't be more help but, like I said, it's been a long
time since I created or edited forms.
