All my drivers are up to date and all Windows Updates have been applied.
I've tried completely uninstalling and re-installing the game, but the
problem persists. Any suggestions on what may be causing the problem?

Same error here. It started this evening.

I was playing the single player campaign for the past week with no
problems and this started a while ago.

I load up select level and difficulty...the game seems to load and then
this error.

I have the same Error on Vista 64 but for me it crashes on the intro before
any menu opens.
