How the hell can I remove file associations?

I think this is Vista's worst 'upgrade' part. Till Windows XP, I could
delete file assocations, add commands and so on. With Windows Vista, I
can only change the default application. What am I supposed to do if I
want to remove obsolete associations to programs that I have already
uninstalled or add secondary commands like Print or Add ..?
