Is there a reference URL that explains *exactly* how FavIcons work in
IE8 and *where/how* they are *normally* stored?

I'm asking about the icons use for the [Start][Favorites] menu.

The favicon.ico file gets downloaded from the site you visit *if* they
make one available. It goes into the TIF (temporary internet files)
folder. If you configure your web browser to clear its cache or use
cleanup tools that do this then you lose the favicon.ico file for this
site. That's why a shortcut can lose its icon because the .ico file no
longer exists. You need to create a folder (e.g., C:\favicons) and
store the favicon.ico files there (just rename them to reflect for which
site they belong, like <sitename>.ico) and update the shortcut so it
gets its icon from there.

Don't expect the contents of icon cache to be permanent.
Win9x/NT4/2K: C:\(Windows|WinNT)\ShellIconCache
WinXP: %userprofile%\Local Settings\Application Data\Iconcache.db
Vista+: %userprofile%\AppData\Local\Iconcache.db
