My Junk mail folder won't empty. It will let me delete individually, but
won't empty on command.

Try repairing the WinMail database: www.oehelp.com/WMUtil/

See if it helps to click on the first item you want to delete,
then hold down the Shift key while scrolling down to the
last one, then release the Shift key and press the Delete
key instead.

Does that method offer any help for deleting items for which
something (perhaps an antivirus program) has deleted the
message files with no attempt to delete them from the index
files as well, and the user has not tried to open the messages
since?

I know it works for length 0 message files, but I've not seen
it work well for non-existant files still listed in the index files.

I've recently deleted over 8000 messages from a Windows
Live Mail folder, after verifying that the corresponding
directory no longer held any message files (only a few
subdirectory files), and had to make an unsuccessful attempt
to open each of these message files before the usual deletion
procedures would work on them.

In other words, WMUtil could use an update to make it
recognize such partially deleted messages, and Windows
Live Mail could use either an update or a WMUtil-
equivalent to do the same.
