Just installed WSUS what about old updates

I just installed WSUS 3.0 SP1 on a 2003 standard server. I synchronized
manually and of course I have almost 1500 updates going way back. Up to this
point, I have been manually approving updates on the server and workstations,
so all machines are pretty much current.

My question is, how can I cut to the chase on what should be approved?
Should I decline the 1500 just downloaded, then approve updates going forward
from here? I have set up my GPO, etc.

What happens if I a approve an update that has already been installed?
