NDISTest failure result for virtual miniport driver

Hi, I developed NDIS 5.1 miniport driver for virtual adapter. it
worked fine most of time. Now I am testing it using NDISTest and got
following:

Is it because I didn't configure the NDISTest correctly or some subtle
bug in the driver? TIA.
