If you downloaded it manually... you don't.
For updates that are published to the Update Catalog, you use the Import
Update function from the WSUS Admin Console.

If the update is not in the Update Catalog, you cannot add it to WSUS
directly. And, if it's not in the update catalog, there's probably a very
good reason for that to be the case.
In this case it looks like it's a six month old Hotfix for Terminal Services
that affects very specific circumstances -- those specific circumstances
being a known, but rarely encountered bug in the Terminal Services device
driver (TermDD.SYS). (There are additional files that will be updated on a
Windows Server 2008 TS environment.)
Now, for one, unless you have =400= TERMINAL SERVERS, you've already
misunderstood the intent of the Hotifx. Specifically, this hotfix is
intended for installation on TERMINAL SERVERS which are explicitly
demonstrating the exact behavior documented in the KB article. Futhermore,
in such situations you should install this update in a supervised manner
(not to mention the fact that you're going to have to restart the Terminal
Server anyway).
