Hello jj and others,
If the service pack has failed to install because the source was required,
this typically means that some Office files have been corrupted or deleted.
In order to restore these files, you will need your Office 2003 CD or network
installation source.
You can fix the problem by following the steps given in this article:

Use the steps under the section titled "Enabling the local installation
source". This will replace certain Office program files with fresh copies
from the CD, and should enable you to successfully install Service Pack 3.
You may also be able to successfully install Service Pack 3 by inserting your
Office 2003 CD into the CD-ROM drive before starting SP3 installation.
The reasons underlying the need for source files are highly complex and
technical. In short, Office needs to reference certain files during
installation of the service pack. If those file are missing or corrupt,
installation fails.
