Sounds like you covered your bases. I really don't see how it's possible
that a HOSTS file was ignored on a workstation, since as I recall that takes
precedence over DNS. Perhaps you needed to clear the browser cache too, or
in addition to the ipconfig /flushdns you mentioned running, also run it
with /registerdns? Does running it with /displaydns show anything
interesting?

Also, I wonder if the reason any of the changes you've made on the server
haven't seemed to have an effect is because it needs the same ipconfig
commands to be run, or maybe even be rebooted?

The fact that this is a VPN might be skewing the situation too, I'm not
sure.
