With USB devices like MP3 players etc an impatient user can cause
problems by unplugging the device without first using the "Safely
Remove" option. I guess that's a bit less likely with a USB drive but
who knows? Are you the only user?
If so then consider whether there could have been a short power outage.
I've seen a few USB "Format" errors created for no apparent reason which
turned out to be cured by using a UPS. Also check the manufacturer's
website for updates to drivers etc. Assuming the ports are on the
motherboard see if you can find anything related to motherboard drivers.
