Recovering lost contacts

I absent-mindedly deleted my microsoft exchange access through activesync
while my phone was connected, and, as it turns out, lost all of my contacts.

I've figured out that to get them back, I must reconnect with microsoft
exchange. But to do that I have to enter server name, user name, password,
and domain. I have no Idea how to find those.

How do I reconnect my phone with microsoft exchange and get my contacts back?

The username and password should be the same you use to logon to your
computer that is connected to the domain that host the Exchange server. As
for the server name, you should contact your system administrator.
