I keep all data on a second drive D: .
Should I need to backup before installing SP3?
And will XP work normally after the upgrade ? [I have only XP on my C: drive]

About as safe as any data can be that is not backed up.
Don't you regularly back up your data? Installing SP3 on C drive will not
affect D drive data.

Any update 'can' kill your machine.
There may be a 99.95% chance it won't, but you never know when you'll be one
of the unlucky 0.05%.
Backing up is always a good idea, especially before a SP.
