Since the Premium Edition includes a second license for Windows Standard
Server, the only way you can install the entire Premium Edition on one server
is to create at least one virtual server.
Since installing the hyper-v role on SBS is not supported, you would be better
served to install the hyper-v role on the Standard Server, then install two
virtual servers, one for SBS the other for Standard Server.
Or, you can simply install SQL on the SBS and not install the Standard Server
that is included with the Premium Edition. Not as a virtual server on the
same box, not as a "real" server on a second box. But if you ever do install
Windows Server Standard on a second box, the best use of that box would be
either for SQL or for TS, or possibly both.
Regardless of how you install SQL, either on the SBS or on the Server Standard
that comes with Premium, you still only need Premium CALS for the users that
actually use SQL. This has nothing to do with the second license, which
could be used, either on metal or inside of a VM, as a file server, print
server terminal server or any other role allowed by Windows Standard Server.
Also, note that however you install SQL, you will need considerably more
RAM than the MS announced minimum of 4 GB, whether you install it directly
on the SBS or in a virtual copy of Windows Standard Server running in a VM.
