session5_initialization_failed Blue Screen after upgrade

Have tried several things to resolve:
booting in VGA mode
booting in safe mode
safe mode with networking, etc. - blue screen everytime
Can get into XP Recovery Console however can't find any boot log to diagnose
the problem. Have tried Michna's boot.ini fix but even that doesn't work.
Anything else to try? Thanks.

Thanks Rich, but nothing there. Most of these I have already tried. Now
looking at a fresh install.

If it similar to this W2K case you should have looked at your version of ntoskrnl.exe

Did that already. Checked several other dll's includding the agp440 file
against 4 other Win XP SP2 upgraded machines that were working fine. All had
the same versions of the files.
