Does SUS have to be in a domain environment? Our office
is all on one big workgroup. Can i setup an SUS server
and have the clients connect to it for updates?

Yes,

SUS does not require being in a domain. However, in order for the clients
to connect to it. You will either have to run a script on all the machines
setting the Server values or apply the local policy to each machine.
