Tracks duplicated when syncing

Using Windows Media Player in Vista with a Sansa Fuze, I find that if I add a
new playlist, tracks that were already copied onto my device are often -- but
not always -- copied a second time with a modified file name. Other times, it
just tells the track is already on my device. Anyone know what might cause
WMP to not recognize tracks that it previously put on my device?
