After 2 days of troubleshooting, I've got my problem narrowed down.
It seems that the UAC is preventing some of my 3rd party shell
extensions from loading. If I disable UAC and reboot, the shell
extensions appear on the context menu. If I re-enable UAC and reboot,
they disappear.

I am running Vista Home Premium 32 bit and I have administrator
rights. Any ideas how I can fix this problem without keeping UAC
disabled?

Hi Jst2swt4,

You can check to see if the programs have a update for them to make
them compatible with Vista. You can also try using compatibility mode
and check "Run this program as administrator" and test it in "Windows XP
(Service Pack 2)" mode. This tutorial will show you how to use
compatibility mode.
