When you click on a non existent file in the recent items, it appears a
dialog box that searches for file and then ask if you want remove the link.
There's a UI bug in that dialog box, because during the search, the icon is
missing and you see a big gray area at the left of text

reproduction steps on Vista SP1:
1. open the start menu -> recent items
2. click on a non existent file
3. while the search is in progress, look that dialog box
.... there's a big area with a missing icon!

That's not a bug. It's showing you the icon associated with the missing
file. If that's your biggest concern with Vista, count your blessings.

What do you expect folks here to do? This isn't an official MS site
for reporting bugs. It's a peer-to-peer site.

BillD's full time job seems to be finding imaginary bugs.

I have a good news: this bug has been fixed by Windows Search 4.0 i.e. when
you try to delete a link to a non existent file, the search in progress
dialog box is no longer shown (so the visual bug has been fixed) and you only
see the second dialog box with a request to remove the file. The whole
process is now very fast because Windows no longer try to search for a file
it already know to be insistent.
