I believe that if McAfee was actively monitoring the system when SP3 was
installed, then that is the most probable cause of the installer failure.

You are referring to the Windows Installer Cleanup Utility, correct ?
Were the registry keys set properly ? Especially the ValueName
'DependOnService' with the Value Data being RpcSs. Without that
setting, the installer service can not run.
And, the ImagePath must point to the location of the installer.
There may be a Permissions issue involved here. That may stem from
McAfee or even malware. Please see this:

The KB article you cite did not recommend unregistering installer,
msiexec, then removing it and related installer files to start with a
clean slate:

See Method 2 to unregister and remove the files.
Now try to install Windows Installer 4.5 once more.
If it fails again, KB315353 suggests doing an in-place upgrade
(reinstallation) of Windows XP -

First, ensure that SP3 has been removed. Go to Start > Run > type in
winver
Click OK or press Enter.
Next to Version 5.1 you'll see the current Service Pack level.
Is it now showing SP2 ? If yes, then you can run the System File Checker
[ sfc /scannow ] :

If no joy, then attempt an in-place upgrade (reinstallation).
