I don't know why you would want to modify one, unless of course you were an
engineer. Treat this as a curiosity unless your anti-virus program notices
it.

Yesterday a curious item appeared in my Start Menu "modify the vista
codecs". I have never seen it before and I'm reluctant to open it because I
don't know if it's potentially hazardous. I right-clicked it and Properties
shows it's an application located in System 32. I Googled "modify the vista
codecs" but very little info is available.

Has anyone seen this? Is it safe? Why would it suddenly show up like that?

It should not. This does not resemble how codecs are managed in Windows
Vista. Dodgy codecs downloaded from the Internet are a favourite carrier of
malware, so your suspicion may very well turn out to be correct.

Please, post in the microsoft.public.windows.vista.security newsgroup for
better advice on malware search and removal. Include information on the
application name, file properties, and your OS/media software setup.
