One possibility is your microphone element is in front of your mouth instead
of off the corner of your mouth. If in front of your mouth, breath pops
(inhales and exhales) are often interpreted as words or commands.
Another tip, whenever speech recognition is not working correctly, redo the
Set up microphone utility ("Peter dictates to his computer......."). This
does several things. First it takes into account the quality of your voice as
your voice is different early in the morning, midday and late at night.
Background noise levels change without your realizing it and stray noise can
be picked up as commands or words. There is also the possibility your
microphone may be at fault (a short circuit or poor noise canceling). At:

When making the recording jiggle the cable of the microphone to see if you
hear static on playback. Also have a radio or TV playing in the background at
a normal listening level. With a good noise canceling microphone you should
not hear the music, only your voice.
