What Instant Messenger would you recommend to a client with SBS2k3

I have a client that wants to set up a Instant Messenger for the office and
she has asked me what I recommend. So now I'm asking what do you use?

Some things I would like to see include:
    1. secure - this client needs to meet federal regulations with HIPAA privacy
    2. universal - would be great if it would work with the big names; MSN, AIM,
    Yahoo, etc.
    3. services - would be great if it offered additional capabilities
    4. FREE - you know how clients are

It would seem to me that #1 and #2 may be mutually exclusive. So I would
    love to hear your suggestions.

    Thanx, David.

  4. Logging.

    Do they provide centralized logging?

Security isn't just about 'security' it's about the records kept of the
    transaction.
